- Use the contact form on this page. The contact form is sent to our Practice Administrator, 24/7, 365 days per week. Our Administrator will forward your message to the appropriate provider
- Access us through MyChart
- Call us at 270-926-CARE (2273). Our three receptionists will usually be registering and checking out patients and cannot answer the telephone. In these situations, an automated attendant will answer with the following message:
- You have reached Premier Medical Group located at 1030 Burlew Boulevard. If this is a medical emergency, please hang up and dial 911.If you know your party's extension, press zero (0).If you are calling from a doctor's office or hospital, press 7.
- Press 1 to speak with a receptionist about an appointment.
